Korean term: 눈밑 꺼짐
What are under-eye hollows
EyeWiki, from the American Academy of Ophthalmology, describes the tear trough as a sunken groove between the lower eyelid and the cheekbone. It can be inherited or come with ageing. Cleveland Clinic says tear troughs cast shadows that make the eyes look puffier.
Hollows are not the same as dark circles, which DermNet says can come from pigment, thin skin or shadow. Cleveland Clinic advises calling a provider if swelling or a dark circle affects only one eye.
Common causes
- Ageing. The skin below the eyes loosens and thins (Cleveland Clinic). The fat under the eye also moves forward as the eyeball sinks slightly, which deepens the groove (EyeWiki).
- Fat, ligament and bone changes. EyeWiki describes a multifactorial process of fat loss, a weakening tear trough ligament and bone loss.
- Inherited anatomy. EyeWiki lists inherited differences, including an underdeveloped upper jawbone.
- Smoking and sun. Both are listed by EyeWiki; DermNet links fat loss around the eye to ageing, genetics and smoking.

- Hyaluronic acid filler. EyeWiki calls it the most common option and gives about 9 to 12 months of improvement on average. A 2025 review describes the ideal candidates as younger people with mild to moderate eye bags, thick skin and little laxity. People with deeper grooves, fat bulges, thin skin or loose skin may see only minor improvement.
- Filler risks. EyeWiki lists bruising, swelling, infection, lumps and migration, and calls a blocked blood vessel the most feared complication. The FDA says injection into a blood vessel can cause tissue death or vision loss. DermNet says improvement may be partial and poor technique can make circles more noticeable.
- Juvelook. One 2025 report followed four Korean women for six months after a single session; severity improved and mild swelling settled within 48 hours. The authors cite the small sample and no control group, and say no large randomized trials show PDLLA lasts longer than hyaluronic acid.
- Other options. DermNet lists fillers or fat grafts for hollowing. Cleveland Clinic says the body absorbs about half the transferred fat.

Questions to ask the clinic
- Is my problem a hollow, a fat bulge, pigment or thin skin, and how did you decide?
- Which product and how much, with a needle or a cannula (a blunt tube), and will you treat the cheek volume first?
- How long should the result last, and when would I repeat it?
- How would you respond to a blocked blood vessel, and whom do I contact after I leave Korea?
- Does the written quote include VAT and a touch-up if the result is lumpy or not as planned?
FAQ
What causes hollows under the eyes?
EyeWiki describes fat loss, a weakening tear trough ligament and bone loss, often with age, on top of inherited anatomy. Smoking and sun exposure are also listed, and Cleveland Clinic says skin below the eyes thins with age. A doctor examines the area to see which factors apply.
How long does under-eye filler last?
EyeWiki gives an average of about 9 to 12 months for hyaluronic acid filler. The FDA says how long a filler lasts depends on the material and the area, and that treatment may need repeating. The only Juvelook report we read followed patients for six months.
Is filler suitable for everyone?
No. A 2025 review says people with deeper grooves, fat bulges, thin skin or loose skin may see only minor improvement, and that existing cheek swelling is generally a reason not to inject the outer groove. A doctor decides after an examination.
What risks does under-eye filler carry?
EyeWiki lists bruising, swelling, infection, lumps and migration. The most serious is injection into a blood vessel, which the FDA says can cause tissue death or vision loss. DermNet says considerable training and experience are required, so ask who injects and how problems are handled.
