Thermage vs Oligio vs Volnewmer: how the three monopolar RF devices differ

Thermage FLX, Oligio and Volnewmer are monopolar radiofrequency (RF) skin-tightening devices that all run at 6.78 MHz. They differ in maker, cooling, the power listed in their FDA filings and the published evidence. Only Thermage FLX's US clearance lists wrinkles. Oligio and Volnewmer are cleared only for electrocoagulation and hemostasis of soft tissue.

A "clearance" is not an approval. Korean launch dates: the first Thermage generation in 2003 (Thermage FLX is the fourth generation), Oligio in June 2020 and Volnewmer in November 2022.

How they differ

Same base technology

All three heat the deeper skin layers with monopolar RF at 6.78 MHz. Solta says the heat tightens existing collagen and stimulates new collagen. The FDA filing for Oligio names Thermage FLX as its predicate device and says Oligio is substantially equivalent to it in design, function and technical characteristics. Volnewmer's filing also uses Thermage FLX as a predicate. This is a regulatory comparison. It does not say the results are the same.

What the FDA clearances say

  • Thermage FLX. Its clearance lists non-invasive treatment of wrinkles, including around the eyes. With skin vibration it adds temporary improvement in the appearance of cellulite. The clearance rested on substantial equivalence to earlier Thermage systems and on bench testing.
  • Oligio, Oligio X and Volnewmer. Their clearances are for electrocoagulation and hemostasis of soft tissue. This is the general RF-device indication and not a specific wrinkle claim. The Oligio filing relied on bench testing, and no clinical study was performed for it. Do not read these as "FDA-cleared for lifting".

Cooling, power and tips

  • Cooling. Thermage FLX cools the skin with R-134A cryogen gas. Volnewmer uses a continuous water-cooling system, which Classys designed to lower pain levels. Oligio uses cooling gas. Its filing lists R-134a, and Oligio X also lists R-1234ZE.
  • Power. The FDA filings list a maximum power of 400 W for Thermage FLX, 145 W for Oligio and 115 W for Volnewmer. These are figures in filings, not a comparison of results. One Oligio study gives 400 W, so we use the FDA figure. The Oligio X filing is inconsistent (140 W in one table and 200 W in its description), so we do not quote it.
  • Tips. Thermage FLX and Volnewmer both list 0.25, 3, 4 and 16 cm² tips. The Thermage sizes come from the Volnewmer filing, not from a Solta document. Oligio and Oligio X list 0.25 and 4.0 cm² tips. Thermage tips are single-use and for one patient only. Volnewmer's filing calls its tip "disposable/reusable", so ask the clinic how tips are handled.
  • Handpiece. The Thermage FLX handpiece vibrates to help comfort, and so does Oligio's, at three levels.

Evidence

  • One head-to-head trial. A randomized split-face trial of 22 women at Severance Hospital compared Volnewmer with Thermage FLX. Both sides were numbed. It was investigator-run, declared no funding and followed patients for 8 weeks. Volnewmer was used in a moving mode and Thermage FLX in the conventional static mode.
  • Pain in that trial. Procedural pain was lower with Volnewmer at every time point. At 300 shots the scores were 4.5 and 8.4 out of 10.
  • Results in that trial. After one session, Volnewmer's results were comparable to Thermage FLX for lifting, skin volume and wrinkles.
  • Limits. The trial is small, from one center, with short follow-up and a different technique on each side.
  • Oligio. We found no head-to-head study of Oligio against Thermage. A 30-patient Thai study reported an average pain of 3.13 out of 10 with numbing cream applied for 1 hour. That number comes from a separate study and is not comparable with the trial above.

Sessions

Solta says one Thermage treatment is usually enough, with results improving over the following 2 to 6 months and lasting 1 to 2 years depending on skin condition and ageing. Most treatments take 30 to 60 minutes depending on the area. That is the maker's statement. The published Oligio and Volnewmer studies used a single session, and we did not find a maker-recommended number of sessions for either.

Which one to ask about

This page does not rank the devices. The doctor decides after an examination. Questions to ask the clinic:

  • Which of these devices does the clinic have? Many clinics have only one.
  • Which version is it: Thermage FLX or an older Thermage, Oligio or Oligio X?
  • Which tip and how many shots does the price include, and for which area? Ask to see the tip packaging and the shot counter.
  • How is pain managed: the cooling method and numbing?
  • How does the clinic describe the FDA status of the device, given the different wording above?
  • How many sessions and what interval does the doctor suggest?
  • Who performs the treatment, and does the price change if the head doctor does it?
  • Is VAT included (부가세 별도 means VAT is charged on top), and is this a regular or an event price?

FAQ

What is the difference between Thermage, Oligio and Volnewmer?

All three are monopolar RF devices at 6.78 MHz. Thermage FLX is made by Solta Medical in the US, Oligio by Wontech and Volnewmer by Classys, both Korean. They differ in cooling, listed power and tips. Only Thermage FLX's US clearance lists wrinkles, while Oligio and Volnewmer are cleared for electrocoagulation and hemostasis.

Is Oligio the same as Thermage?

No. Both are monopolar RF devices at 6.78 MHz, and Oligio's FDA filing names Thermage FLX as a comparison device. But they come from different makers and differ in cooling, listed power (145 W for Oligio and 400 W for Thermage FLX) and tips. We found no head-to-head study of the two.

Which of the three is less painful?

Only one trial compared pain head to head. In 22 women with both sides numbed, pain was lower with Volnewmer than with Thermage FLX at every time point, though the two sides used different techniques. We found no head-to-head pain data for Oligio. Pain also depends on the energy, the area and the numbing used.

Are Oligio and Volnewmer FDA approved for skin lifting?

No. Both are FDA-cleared, not approved, and the clearances are for electrocoagulation and hemostasis of soft tissue, not for lifting or wrinkles. Of the three, only Thermage FLX's clearance lists wrinkles. Ask the clinic how it describes the FDA status of its device.

How many sessions of Thermage, Oligio or Volnewmer do I need?

Solta says one Thermage treatment is usually enough, with results improving over 2 to 6 months. That is the maker's statement. The published Oligio and Volnewmer studies used one session, and we did not find a maker-recommended number for either. Your doctor decides after an examination.
